Two identification documents:

1. One document must mention your name and your date of birth:
   • birth certificate;
   • health insurance card;
   • certificate of citizenship;
   • etc.
2. The other document must indicate your name and your address:
   • driver's licence;
   • telephone bill ;
   • electricity bill;
   • etc.
